Four other people have been taken to hospital after the horror smash in Glenbervie, Aberdeenshire, at about 4.30pm on Tuesday.
Police Scotland said the crash involved a red Renault Megane and a silver Ford B Max and a Megabus coach.
One person had to be airlifted to hospital while three others were taken by ambulance to Aberdeen Royal Infirmary.
Their injuries are described as serious but not life-threatening.
The bus driver sustained minor injuries, whilst all of his passengers escaped unharmed.
It was absolute carnage. There was people on the road that we couldn't help.
Road Policing Chief Inspector Stewart Mackie said: “This has been a challenging incident for the emergency services to deal with and my thoughts are with the families of all those affected by this.
“I would ask that anyone who witnessed the incident and who hasn’t already spoken to the police contacts officers on 101 using reference number 2576 of 12 March.
“The road is anticipated to remain closed for some time to allow collision investigation to take place.
"This is likely to be a complex enquiry that will take some time and I am grateful to the public for their patience while this is carried out. Further details will follow once they are available."
Eyewitness Jillie Aitchison, 45, was behind the pile-up and stopped to give first aid at the scene before emergency services arrived.
She told Northsound: "I was travelling northbound in the car with my daughter.
"It was absolute carnage. There was people on the road that we couldn't help. I tried to administer first aid."
The road has been closed in both directions and police expect it to be shut for some time.
Around 25 firefighters were at the scene with cutting gear freeing those trapped in their vehicles.
