The Belgium midfielder hobbled off helped by Spurs physios before being carried around the pitch for treatment.
The 31-year-old was instantly replaced by Son Heung-min at Molineux, forcing Mauricio Pochettino into a slight formation tweak early on.
His whole leg was strapped up, but it is understood he has suffered an ankle injury, the extend of which is not known.
It is thought the only touch he had was for the kick-off of the game.
