The pair tied the knot in Portugal in September, with Jeff's two sons with the late Jade Goody his best men at the ceremony.
Taking to Twitter, Jeff shared a snap of his bride grinning on their wedding day as he delivered a speech.
The reality star captioned the photo: "Ah we met on our way to Mauritius for work five years ago today!
"Our hard work has truly paid off. All you can ever hope for is that we keep making each other smile like this.
"Here’s to another five."
Posting the same photo on Instagram, Jeff wrote: "Little while ago I met this treacle on the perfect work trip and I instantly knew she was in trouble.
"Five years later she’s my Mrs and she hasn’t seen my ‘A game’ since!
"Luckily my B game is half decent too."
Kate also marked the anniversary with her own unseen wedding photo, tweeting: "This time five years ago I was sat on a plane next to @JeffBrazier trying to act normal when inside I was totally losing my s***.
"Now I’m heading home to him feeling so #grateful that I get to do that every day."
Another image on her Instagram page was captioned: "FIVE years ago today this man walked into my life.
"He’s my husband, my biggest supporter, the best teammate and I can’t even remember what life was about before."
The anniversary posts come just days after Kate shared the moment that Jeff's sons gave their touching best man speeches to her Instagram page.
The brunette beauty opened up about being stepmum to Bobby, 15, and Freddie, 14 – who lost their mum to cervical cancer in 2009.
She captioned the photo: "Because all my #tbts are wedding ones.
"There are good days and bad days and in all honesty I am still learning to be better every single day of this #stepmum journey.
"But, this was without a doubt the single most special moment of my life with these boys so far.
"The only opinions that matter are theirs and to see them stand up and tell everyone that I am getting it right (most of the time) was nothing short of life affirming for me.
"What an absolute honour."
Jeff previously admitted that his children were desperate for a mother figure, with him raising them on his own since they were young.
Jeff began dating late reality star Jade in 2002, with the pair splitting two years later – shortly after Freddie's birth.
He gained full custody of the boys after Jade's passing nine years ago, and started dating Kate in 2013.
The pair got engaged in Ibiza in 2017 before tying the knot in September.
In an interview with Hello! Jeff previously said of his marriage: "This isn't just any wedding, this is a really special one.
"It doesn't just represent Kate and I putting rings on each other's fingers. It represents the fact that we won at something really tricky. We won at becoming a family."
He added: "Before it was just three boys rattling around in a house on our own, trying to get through life. Then Kate came along and worked so hard at making us complete."
