A man claims he was left bleeding from his mouth after he bit on glass shards while eating a pizza at ASK Italian.
Josh Davies, from Gower, South Wales, claims he could feel glass stuck in his teeth after a "crunch and sharp sensation" in his mouth.
The 27-year-old took his girlfriend to the restaurant in Swansea on April 12 to celebrate their anniversary.
He told WalesOnline: "I ingested it unknowingly. I took another bite with the same sensation and to my disbelief I spat it out as I could taste blood.
"As I looked through the pizza, there were glass shards in there."
Josh claims he approached a waitress and was then met by the restaurant manager who apologised and said the meal would not be charged.
He added: "When I went back to the bathroom I had to find some string on my hoodie to get the glass out of my teeth as my mouth was bleeding, so I used a tissue to try and stop it."
Josh claims it was difficult to get in touch with the manager for an explanation following the incident.
He said he was initially told by the manager that the staff at head office would contact him the next day. But he waited for three days and got no reply.
Eventually, he went to the restaurant and asked for the manager, he said.
Josh claimed: "The manager apologised. He said he would contact me in a week. A further three days later I had no phone call and again I had to call back.
"To my surprise, the manager was not there. I left a message to the assistant manager who referred to me as 'the glass man' and that was absolutely jaw-dropping."
Josh, who claimed he suffers from anxiety and is on medication for it, said he didn't hear anything for five weeks until they sent him a gift card.
He added: "Not only have I had to chase this up and try to get it resolved but it's just beyond a joke.
"A free meal and a gift card doesn't cover it, I don't go out to a restaurant to eat glass."
A spokesman for ASK Italian said: "We apologise for this, and any additional anxiety that this may have caused Mr Davies. We have very strict controls in place to prevent such incidents from occurring including a strict ‘no glass’ policy in our kitchens and all our ingredients coming from certified suppliers.
"In April, we did send Mr Davies our sincerest apologies along with a full summary of our investigation and a gift voucher.
"This was two weeks after the incident. We accept, and are sorry, that Mr Davies didn’t seem to receive our correspondence, just our gesture of goodwill, so our Head of Operations will be contacting him personally.
"Once again, we very much regret that our communication to Mr Davies was substandard, and reiterate our apologies to him for this distressing but isolated incident."
